The Facility has a great staff on days and night. The down side is the staff minimal required personnel which means they are over worked. The food is a mystery ( unseasoned unidentifiable mush plates) every meal unless you request a sandwich. The building needs a deep clean if they aren’t going to direct funds to repairs and updating the facility. Patients are every where. Speaking of the patients the building has no check points not sure if that’s protocol patients are wondering unsupervised which could potentially cause an incident or injury. The parking lot there aren’t any spaces for visitor’s to park. Which I can’t blame them I wouldn’t want family members to see the worn down building. Any how the visitors and staff park in the same approximately 30 spots. You have to park at the emergency room behind the facility and walk to visit handicapped or not. Please check in on your love ones at different times of the day. Please visit your loved ones and make sure their basic needs are met because sometimes the hot water works sometimes it doesn’t. The laundry was having issues also no washer or dryer at the facility was working properly. Please advocate for your loved ones especially the ones that can’t communicate properly. I gave it two stars because the staff is good.

Azalea Health has done a great job seeing to my family member’s needs. In particular the nursing staff and the director Stephanie have done a great job keeping me informed since I do not live locally. They are always helpful and available when I reach out to check in. They seem to really care about the well being of those that are staying at the center.

My name is Juan Barkley and I'm a resident at Azalea Health and Rehab. Mary Pixley took very good care of me while I was on her assignment She's very professional and very prompt with her care taking. I would like for her to get recognized for her the good work she performs Beverly Fuller and Quemecka Callaway are the best wound care nurses I have ever had. I came from Florida and have been treated at other places in Augusta and these two ladies were able to get my wounds healed.

Nurse high I recorded her and per an employee facility is aware and has had other complaints and still working with patients! This is a disgrace Unethical, unprofessional, and scary I have videos unable to load here only photos

My brother Eddie Holmes has been a patient at Azalea Healthcare center for approx.a year .He was admitted with a severe bed sore.Thanks to Ms.Qumecka Callaway and Mrs Beverly Fuller, which are the wound care nurses, he's on the road of healing. These two ladies has done a wonderful job taking care of my brother..... On behalf of himself and my entire family we would like to say thank you both.
